Bio:

J. R. Fitzpatrick is undoubtedly the same man as Jefferson Thomas Fitzpatrick (who was also recorded as J. T. and, undoubtedly a transcription error, as J. F).
There was apparently confusion at the V.A. as to the correct name as it was used incorrectly on more than one document. This man was assigned to the same company and regiment, he died on the same day, etc.
